Reporting a Work-Related Injury/Illness (Employees)

For all injuries/illnesses, employees need to complete an “Employee/Volunteer Injury/Illness Report” form. Complete and sign the “Employee” portion and return it to your manager or supervisor.

This form is available in your department office, or on this web-site under the heading “forms” and or by calling Risk Management extensions 7346 or 2824.
