To protect the identity and reputation of individuals accused
of committing improper governmental activities or causing a significant
threat to health or safety, the University does not undertake formal
investigations without having a basis for believing there is merit
to a complaint. For this reason, please include all evidence you
have that leads you to believe that an improper governmental activity
has occurred or that a significant threat to health or safety exists.
When describing the improper activities/health or safety threat
about which you are making a disclosure, please number each allegation
and provide the following information for each allegation.
| Who is involved? If there are businesses involved,
what are the names of those businesses, who owns them, and where
are they located? Who else knows about the improper activities/health
or safety threat? Who can and would confirm that they occurred?
How can we reach these witnesses? |
