The University has developed the following form to
transfer the risks associated with an activity or event to the
participant in exchange for allowing participation.
- "Release
of Liability:" For use with all activities, both mandatory and
voluntary, including those that involve air travel or physical
activity.
Releases are also not required to participate in or attend a:
- Class activity
- Campus tour
- Cultural performance or athletic event
- Meeting or other function on campus
Releases are also not required to participate in self-selected
academic or club activity. For example, when a faculty member
requires a student to visit a museum or attend a performance as a
class requirement but gives the student the freedom to decide what
museum to visit/performance to attend and when. Please use this
matrix to determine whether a release is required.
Instructions
Prior to use, copy the appropriate document on University
letterhead and insert the name (with a brief description), date and
location of the activity. Do not alter the document in any other
way. Information for participants including preparation,
equipment and safety should be provided on a separate document.
Provide ample opportunity for the participant to review the
document prior to the beginning of the activity or the event. Do
not permit anyone to participate in the activity who has not signed
the document or who has altered the document in any way before
signing. A parent or guardian must sign if the participant is
under 18 years of age.
Keep the signed documents on file for at least three years after
the date of the activity. If the participant was a minor, keep
the document until three years after minor turns
eighteen.
