Whistleblower Retaliation Complaints

To protect the identity and reputation of individuals accused of retaliating against an employee or applicant for employment for making a disclosure of improper governmental activities or significant threats to health or safety, the University does not undertake formal investigations without having a basis for believing there is merit to a complaint. For this reason, please include all evidence you have that leads you to believe that you have been retaliated against for making a disclosure under Executive Order 929 or directly to the State Auditor.

When describing the retaliation, please number each allegation and provide the following information for each allegation.

  Who? Who is involved? Who else knows about the retaliation? Who can and would confirm that it occurred? How can we reach these witnesses?
     
  What? What specifically happened? What is wrong with it? What kinds of documents would provide evidence of the retaliation? Where are the documents located? Who controls them?
     
  Where? Which department? Which division? What location (address, city)?
     
  When? When did the retaliation occur? Is it ongoing? How frequently has it occurred?
     
  Why? Why did the retaliation occur?
     
  How? How did the individual you allege retaliated against you know that you made a disclosure under EO 929 or directly to the State Auditor?
